As a long time diver and longer time ear-infection sufferer (and owner of a Sea&Sea, the MX-10) I'll give you a piece of advice that cured my recurring ear infections forever.
Every night after a day of diving I put a few drops of "Merthiolate" (the white, not the coloured variety!) on each ear, let it sit for a couple of seconds and then let it run out. I've even used alcohol in a pinch, but the Merthiolate works really well.
Some doctors have told me it dries up the inside of the ear, some others have told me that it's really mild and won't affect anything.
For one thing it stopped the ear infections I used to get every single time I swam in the ocean. Of course when I took up diving I returned from my first trip with a C-Card and a fat ear. A fellow diver gave me that tip and I haven't suffered an ear infection ever since.
Other than that I can only tell you to buy the widest lens you can find for your D200 and shoot from the bottom pointing towards the surface, but I guess you knew that already.
