Price Quote/Questionairre template for website

I've seen many websites that have an area for the bride and groom to fill out and then they click "send" and it sends to the photographer's e-mail. Anyone know where the html for something like this could be found, or how to get such a template?

You will find many samples while looking for "html mail form" at google, this page: http://www.htmlgoodies.com/tutorials/forms/article.php/3479121 explains how to do it easily, there are many ways to get the same.
